To be considered for the Mi2 Forever Sports Scholarship, please fill out the application below.

Once your application has been submitted please allow up to 2 weeks for a decision. If you have been selected to receive the scholarship, you will be contacted by the Mi2 Forever Foundation.

If you are a coach, sports league, school staff, mentor or from another outside agency and would like to apply on behalf of an athlete, please do not fill out this application. Email donations@mi2forever.org. Please include the subject: Mi2 Forever $400 Sports Scholarship. Also include your contact information, your organization and your role, the child’s name, age, sport and the reason you are requesting the scholarship on behalf of this child.
